WHAT TO USE TO COMPLETE THE FORMS 2024/2025

For the validity of the registration forms, the completion and signatures in all its parts are required. The forms must be delivered in person to the President or to M. Mauro Corato or by certified mail to the address shinzenbi@pec.shinzenbi.it .

Safeguarding Material to Read(Required)

 

From this year the association has adapted to the safeguarding regulations and has prepared all the necessary documentation.

It has also created a protocol for managing reports, to ensure maximum control and confidentiality for everyone.

We trust in your collaboration as not knowing the protocols does not exclude you from civil or criminal disputes.

WINDOWS
Open the Acrobat Reader DC executable obtained and, in the window that appears on the desktop, click on the Yes button; then wait for the download of the components to make the software work complete and click on the Finish button.

MacOS
Open the installation package obtained and double-click on the .pkg file in the window that appears. Then, press the Continue button (twice consecutively), type the password for your user account on the Mac, click the Install software button and complete the setup by clicking the Close button.

 

PROCEDURE FOR COMPLETING THE FORM

The forms are equipped with an advanced signature to make your signatures validated
 

WAYS TO SIGN

You can decide to obtain the signature to be inserted on the document:

  • typing it with the keyboard
  • drawing it with the mouse or touchpad
  • by uploading the relevant image

Click the Signature icon in the toolbar. Alternatively, you can choose Tools > Fill & Sign, or choose Fill & Sign from the right pane.
Sign in the designated spaces and save the signed form by clicking on the File menu located at the top left and choosing Save as from there to create a new file.

 

In advanced form:

  • Using signature creation device
  • Using a digital ID
  • By creating a digital ID
 

Click on the point of the document where you want to add the signature, resize it if necessary using the appropriate cursor and save the signed PDF by clicking on the File menu located at the top left and choosing from the latter the Save as item, in to create a new file. Perform this operation for each signature (For the minor form 3 Signatures for Parents 1 and 2 Signatures for Parent 2).

 

PROCEDURE FOR PERSONAL DELIVERY

  • Registration Form

  • Copy of identity documents (To register minors, attach copies of both parents as well as that of the minor)
  • Sports Medical Certificate in original form. (For competitive certificate, provide a copy of the Association)

 

Please remember that anyone who delivers the documentation by hand must send a copy of the payment to the email address amministrazione@shinzenbi.it

 

PROCEDURE FOR SENDING BY PEC MAIL

  • Copy of identity documents (To register minors, attach copies of both parents as well as that of the minor)

Please remember that the Sports Medical Certificate must be delivered in its original form.

 

ADDITIONAL INFORMATION

TYPES OF DIGITAL SIGNATURES

There are various types of electronic signatures, each with a different level of legal validity.


ELECTRONIC SIGNATURE
It is a simple graphic reproduction of the signature which has no intrinsic legal value, considering that there are no methods of electronic recognition of the same. A judge must decide on its legal validity, who evaluates its authenticity on a case-by-case basis.

ADVANCED ELECTRONIC SIGNATURE
It is a type of company created by certain software. It has full legal value, except for real estate contracts. The signatory has direct and exclusive control of the signature and can demonstrate its authenticity using a device of his own.

QUALIFIED ELECTRONICS COMPANY OR DIGITAL COMPANY
It is the most advanced type of electronic signature, it is created through specific signature kits on sale by recognized certification bodies. The legal value is full and the verification of authenticity is guaranteed by a signature system encrypted with a public key.

 

VALIDITY OF A DIGITAL SIGNATURE

In our legal system, a document signed with a digital signature has full legal effect if the following conditions are respected:

  • The digital signature is affixed using a signature certificate issued by an accredited body capable of positively identifying the owner.
  • The digital signature is affixed using a valid signature certificate.
  • The document is not modified after the signature has been affixed.

 

If even one of these three conditions is missing, the digital signature is NOT valid ..


CONDITIONS IN DETAIL


SIGNATURE CERTIFICATE ISSUED BY AN ACCREDITED BODY
The fact that the digital signature must be affixed using a signature certificate issued by an accredited body capable of positively identifying the owner means that by analyzing a document with a valid digital signature it is possible to verify the identity of the person who signed it in a certain way (this is called authentication of the signatory).


VALID SIGNATURE CERTIFICATE
The fact that the digital signature is affixed using a valid signature certificate means that the signatory of a document with a valid digital signature cannot deny the document he signed, unless he provides proof contrary (this is called non-repudiation of the document). This happens because the signature certificate has a limited validity in time, and it is believed that a possible attacker cannot identify the password necessary for using the certificate before the expiration of this last.
Furthermore, the digital signature placed on a document using a valid certificate no longer has any value after the expiration of the certificate itself unless a timestamp is placed on the document before the expiration of the certificate used for the signature. Please bear in mind, however, that uploading a digitally signed document into a compliant protocol system is equivalent in all respects to the affixing of a time stamp and therefore, regularly registered digitally signed documents remain valid even after the expiry of the certificate used for signing
Finally, from the fact that a signatory cannot deny a document digitally signed by him, it follows the importance of keeping the password associated with his signature certificate secret, so as not to allow anyone to add digital signatures on his behalf.


DOCUMENT CANNOT BE MODIFIED AFTER SIGNATURE
The fact that the document cannot be modified after the digital signature has been affixed means that a document with a valid digital signature is identical to the one at the time of signature (this is referred to as the integrity of the document).
Given that a minimal modification at any point of the document would lead to the invalidity of the digital signature, in the case of digitally signed documents the act of initialing the various pages of a document no longer makes sense (if we want to draw a parallel with paper, we can consider it as if the digital document were written entirely on a single sheet).
Since digitally signed documents no longer need to be modified after the digital signature has been affixed, we recall the general ruleto sign only files in PDF format and, where possible.

 

DIFFERENCES BETWEEN CAdES AND PAdES SIGNATURES

CAdES SIGNATURES
In the case of a digital signature affixed using CAdES mode, the signed document and the file with the digital signature are inserted together in an envelope. This envelope, which contains the document and the signature file, is also a file with the .p7m extension. In fact, all files digitally signed with CAdES mode have a second extension.p7m.  

Let's see the main characteristics of this signature method:

  • The CAdES mode allows you to sign any type of document (docx, .xlsx, etc.), although we still recommend signing files in .pdf format.
  • A document, once signed with CAdES modality, changes its name. For example, a Prova.docx document, once digitally signed with CAdES mode, will change its name to Prova.docx.p7m.
  • To verify a digital signature affixed with CAdES mode and to view the signed document, you need to use one of the appropriate specific software such as Dike 6, ArubaSign, etc.

 

PAdES SIGNATURE
In the case of a digital signature affixed with PAdES mode, however, the characteristics of documents in .pdf format are exploited and the file containing the digital signature is incorporated together with the document itself.

Let's see the main characteristics of this signature method:

  • The PAdES mode allows you to sign only documents in .pdf format
  • A document, once signed with PAdES mode, retains its name
  • To verify a digital signature affixed with PAdES mode and to view the signed document, you can use any software for reading .pdf files such as Acrobat Reader.

 

SPORTING FITNESS CERTIFICATE

In Italy there are 3 types of sports certificates:

  • COMPETITIVE  (Age varies based on sport)
  • NON-COMPETITIVE  (Obligatory from 6 years)
  • PLAY-MOTOR  (Optional from 2013)


CERTIFICATE OF COMPETITIVE SUITABILITY

Those who practice sports and are members of sports clubs affiliated with the National Federation or with a Sports Promotion Body recognized by CONI or CIP are considered competitive. Pursuant to the Ministerial Decree 02.18.1982 and the Ministerial Decree 03.04.1993, competitive athletes must necessarily possess the certificate of competitive sports suitability.

 

THERE ARE 2 LEVELS OF VISIT IN RELATION TO SPORT FOR WHICH SUITABILITY IS REQUIRED

  • "Type B" for highly demanding cardio-vascular activities, which includes family and personal anamnesis, complete physical examination, spirographic evaluation of the main respiratory and ventilatory parameters, resting and exercise electrocardiogram and urine test (for example: football, volleyball, basketball, cycling, swimming, athletics)
  • "Type A" for sports activities with low cardio-circulatory commitment which, compared to the type "B" visit, does not include the ECG after exercise and the spirometric test (motor racing, bobsleigh, sledding, bowls, curling, golf, karting , speed motorcycling, powerboating, tambourine, shooting, skeet shooting, archery)

Competitive athletes must present themselves with the request of the Sports Club to which they belong drawn up on a regional form, stamped and signed in original by the President of the Club.(REQUEST THE FORM AT THE GYM)
Minor athletes must necessarily be accompanied by a parent. Only in case of actual impossibility can another family member be delegated. The delegated person must be able to give consent to carrying out the checks, sign the medical history and sign the consent to the processing of data.

 

The competitive fitness check is free for minor athletes and for disabled athletes (with a disability greater than 66%); for all the others there is a fee, based on the sport practiced.

 


NON-COMPETITIVE CERTIFICATE OF SUITABILITY

At the Sports Medicine Facilities it is possible to obtain (for a fee) the non-competitive sports fitness certificates based on what is determined by the Ministerial Decree 04/24/2013 (Balduzzi Decree) and is mandatory for people over the age of 6.
Carrying out sporting activities at a non-competitive level requires a medical examination to verify the state of good health as well as the execution or viewing of the basic electrocardiogram (ECG).


CERTIFICATE OF PLAY-MOTOR SUITABILITY

The Certificate for recreational-motor sports activity is a certificate that is no longer mandatory but optional since 2013
(Ministerial Law no. 98 of 9 August 2013) and is issued upon request to anyone who practices non-competitive sports and who is not registered with the National Sports Federations or by Sports Promotion Bodies recognized by CONI.

 

DO YOU NEED A CERTIFICATE FROM 0 TO 6 YEARS?

There is no obligation to present a medical certificate for sporting activities carried out by children aged 0 to 6 years, except for cases reported by the paediatrician. This is provided for by a decree issued by the Ministry of Health in March 2018 in agreement with the Ministry of Sport and which concerns sports practiced in pre-school age.

  COMPLAINT FORMS RACE (To be submitted within 30 minutes of the end of the race)

The final decision decrees the conclusion of the match. Any changes can no longer be made at the time of the match but must be discussed afterwards.

In the event of an APPEAL, the Team of the athlete who intends to contest the verdict must report within 30 minutes of the end of the match, using the contestation forms. The contestation must be delivered to the President of the Jury who will then deliver it to the central table that will examine the contestation.

The intention to appeal against the verdict will entail an investigation fee of € 30.00 which, only in the event of an accepted appeal, will be returned net of any expenses incurred by the examiners.

Possible irregularities:

  • Inconsistency between the final verdict and the results of the bulletins (taking into account the penalties).
  • Errors in the compilation of the bulletins by the judges that have determined an irregular verdict compared to the actual progress of the match.
  • Determination of a clear violation of the rules and regulations that have determined a different verdict.
  • The Judge's discretion to express his opinion on the card cannot be the basis for appeal.

  Certificate of fitness for not competitive sporting activity:

BASIC ELECTROCARDIOGRAM MANDATORY

For those who have reached the age of 40, the supplementary certificate with electrocardigram under stress is also required.

    WITHOUT THE REQUIRED MEDICAL CERTIFICATION YOU WILL NOT BE ABLE TO SUPPORT THE TRAININGS

 

The TEST lesson can also be taken without presenting the medical certificate but will be of reduced intensity.

 

Certificate of fitness for competitive sporting activity

COMPETITIVE ATHLETES MUST PRESENT THEMSELVES WITH THE REQUEST OF THE SPORT ASSOCIATION TO WHICH THEY BELONG, DRAWN UP ON A REGIONAL FORM, STAMPED AND SIGNED IN ORIGINAL BY PRESIDENT OF THE ASSOCATION

The competitive fitness visit is free for minor athletes and for disabled athletes (with invalidity greater than 66%); for all others it is chargeable, based on the sport practiced.

 

Certificate of Refereeing Eligibility for the Circuit:

To be able to referee, it is necessary to have a non-competitive certificate.

The fitness visit is entirely the responsibility of the person who intends to join the refereeing body.
